Due to New York State Law & Total Tan policy:

• All clients will be required to sign a consent form every 12 months.

• All clients between the ages of 14 and 17 will be required to have a consent form signed by a parent or guardian, in the presence of a Total Tan employee, every 12 months.
  (Parent or guardian must be present. Notes from parent or guardian will not be accepted)

• All clients will be required to show a valid photo ID verifying their identity and date of birth at EACH visit.

• All clients will be required to show FDA approved eyewear at EACH visit.

• No person under the age of 14 may tan in UV tanning units.

• Clients under the age of 14 may use our UV-Free Mystic Tan unit. (Parent or guardian must be present.)
